Have you had support from a Social Prescriber?
The Patients Association and The National Academy for Social Prescribing (NASP) want to hear from patients who have had support from social prescribing link workers.
They have produced a survey to learn about patient experiences to contribute to NASP's new strategy to support the provision of of the best and most appropriate social prescribing services for all.
Social prescribing link workers spend time with a patient to understand their needs and co-develops a plan with them to connect to non-medical support, improving their health and wellbeing, and to their community.
